When renal 1α-hydroxylase is impaired, ordinary vitamin D3 struggles to become calcitriol; alfacalcidol is already hydroxylated at position 1 and only needs the liver to finish 25-hydroxylation. The active hormone then induces intestinal calcium-binding protein, raising calcium and phosphate absorption so osteoid can mineralize; bone density and blood calcium are markers often watched together.
Maintains calcium–phosphate balance
Calcitriol regulates the parathyroid; trials in dialysis-related bone disease look at parathyroid hormone and blood calcium and phosphate. To understand the molecule, treat it as a vitamin D hormone precursor that skips the renal 1α step; bone and parathyroid both receive the same active hormone.
