Supplies the rate-limiting thiol for glutathione
After oral absorption, NAC is deacetylated in the gut and liver to cysteine, which γ-glutamylcysteine synthetase uses with glutamate and glycine to make glutathione. Cells use this thiol to keep reducing power and conjugate electrophiles. For hepatic oxidative load, the use is precursor supply. Hospital high-dose treatment of paracetamol overdose is a prescription setting, not a wellness dose.
Breaks mucin disulfide bonds
Free thiols can open disulfide bonds in airway mucins, thinning sputum so cilia can move it more easily. That is a rheology effect of an exogenous acetylated thiol, not cysteine being coded into protein. It fits cough with thick, hard-to-clear mucus. NAC smells sulfurous; some people feel nauseous on an empty stomach and can take it with food.

Do not take with activated charcoal; splitting the dose can improve stomach tolerance.
