D-mannose is a monosaccharide. After it enters the body, most of it reaches urine unchanged via the kidney, occupying mannose-binding sites on Escherichia coli fimbriae and supporting uroepithelial comfort.

Mannose's stereochemistry matches the binding protein of E. coli type 1 fimbriae. Free mannose in urine occupies these sites first, so bacteria have a harder time sticking to bladder epithelium. People with repeated frequency and burning usually feel this as lower-urinary-tract comfort.
